What to expect at your first visit...

For your initial appointment, please come 15-20 minutes early to complete all necessary paperwork
.
We encourage you to download new patient forms listed below and complete them beforehand.

Please note: All pain scales must be completed within 24 hours of your first visit.

Please bring your insurance card and your doctor's order for physical therapy, if one was given to you.

Please dress with the proper work out attire for your injury, wear comfortable/enclosed shoes, and if attending aquatic therapy, please bring an appropriate swimsuit.

Bring your calendar/day planner so that you may schedule appointments in advance.

Insurance______________________________________________

If you are a patient interested in receiving physical therapy from our clinic, you do not need a referral in order to be treated. However, you might need a referral to comply with your insurance coverage.

We participate in most insurance plans, including Medicare, and file insurance claims for our patients with participating providers.

If you are a physician or healthcare provider and you would like to refer a patient to our clinic, we would be happy to send you prescription pads for your convenience. Simply email your request to info@dunnpt.com.
